These forms
will be pre-populated with the student's information provided
during the enrollment process and available for parent review
and signature at the parent meeting on June 4, 2009.
If you will
not be attending the parent meeting, please complete each form,
sign, and return the completed forms to the Summer Academy
office before the first day of camp.
